This is a free service to anyone age birth through age 20 that is eligible for Ohio Medicaid.

Well-child visits are important to raise healthy kids:

  • Preventative health care to monitor growth and development
  • Early treatment if there is a problem

What is included?

  • Complete physical
  • Eye exam
  • Hearing test
  • Dental check (not cleaning)
  • Immunizations (if needed)
  • Lead level test (if needed)
  • Other testing, health education, behavioral health and other rehabilitative services

Healthchek covers up to 8 check-ups until the child is 2 years old, and then annually each year thereafter.

The Healthchek Coordinator can also help with other social services which may include:  Tracking appointments and when immunizations are due, finding a doctor, scheduling appointments,  arranging transportation and addressing many other needs.

                                                                                                             

Pregnancy Related Services                                                                pregnancypic.gif

                                            

Eligibility begins from the date suspected pregnancy is reported to the agency and continues through 60 days after the end of the pregnancy. The infant remains eligible for Prenatal Transportation for up to one year.

What is the purpose?

  • Early prenatal care leads to healthy babies
  • A healthy mom is more likely to have less complications
  • During the visits, the doctor may be able to identify and reduce the risk factors or dangers to the mother and/or child.

What is covered?

  • Prenatal exams and testing
  • Transportation to medical appointments
  • Nutrition counseling
  • Fetal delivery
  • Education on caring for the infant
  • Pre-birth visit to the pediatrician

The PRS worker can assist in finding a physician for prenatal care and delivery,  help with finding a pediatrician for the newborn, assist in scheduling appointments,  arrange transportation to medical appointments, and assist with other services, such as:  referral for other social services, clothing, housing, etc.
